Development of medications administration model for safety in the in-patient department at a community hospital in Roi-Et province

Abstract

This action research to develop of medication administration model for safety, and to evaluate the performance of nurses practicing medication administration in IPD at a community hospital in Roi-Et Province. Participated divide into 8 executive nurses and 11 operational nurses. The quantitative consisted of a checklist and observation form of nurses for medication administration. The qualitative consisted of group discussion to find key issues and guidelines for the development of medication administration. Models. Quantitative data were analyzed using percentage, mean, standard deviation and Wilcoxon Matched pairs signed Rank test. The qualitative data synthesized the medication administration model by content analysis. 
The results showed that medication administration model for safety in IPD according to the 10R and 3 process: Pre-administration, during administration, and after administration. Get a suitable for the context. Cycle one, it was found that self-assessment of nurses passed criteria 72.73% and the executive nurses observed passed criteria 77.27%. Adjusted in the second cycle preparing by 1) Medical order sign 2) Medication labels using a computer 3) Parameters for HAD 4) Warning signs to communicate with patients and relatives 5) Reflecting the problems and allocating more power. The medication administration models have been developed and improved, and administration errors decreased from 2.01 to 0.78 per thousand bedtimes. The observational results of the medication administration practices of nurses pass the criteria of 80%. Self-evaluation of nurses' practice after using the improved medication administration model. It was found that nurses' practice for patient safety after using the model was higher than before statistical significance at the 0.05 level.
